Despite being a Wife, Esther had been taken to the Red Centre to become a Handmaid after it was exposed that she had been hiding the fugitive women in her house.
There she rebelled against Aunt Lydia and refused to eat, however, Janine was able to convince her as the punishment would be a lot worse.
Fans will have to tune into season five to see if this theory comes true, perhaps Janine could use her role as an aunt to escape successfully.
