This is a medical supply train, which travels around the country and offers out medical advice and treatments.

The storyline is based on a real service in India called the Lifeline Express, which has been running since 1991.

Speaking about its inclusion in the show at a recent screening, Dr Gabriel Varma actor James Floyd told Express.co.uk and other media about its inspiration.

He said: “It’s basically a train that travels through all parts of India, but in this case south India coming from the north.

“It will do a bunch of operations that the local hospitals won’t be able to do.”

The Good Karma Hospital is also known for utilising many local workers when it films in Sri Lanka.

This was confirmed by director Philip John, who explained how they had used around 600 locals to be background extras.

He said of filming on location: “It was a life changing thing, for me to go there.

“You just see the poverty and they’ve been through so much, they’ve had the civil war, they’ve had the tsunami and the place looks like it’s been trashed for 50 years, the infrastructure’s a bit c**p and nothing really works that well.”

However, he added: “But the people are so happy and it changed me. I want to go back.”

Many of the cast also love filming in Sri Lanka, including Greg McConnell actor Neil Morrissey who spoke about the experience at the screening.
