At the time, the coroner submitted a “Report to Prevent Further Deaths” to local authorities and the club that said “a risk of further deaths would occur unless action is taken”.

She said: “I could not say that this will not happen again — I just don’t know.
“I couldn’t say they have not taken the matter seriously because they have. But whether they have taken the right steps I could not say.
“They will think that they have provided assurances but we cannot ignore what happened. How confident are they?
“It is for them to demonstrate that their procedures are adequate.”
United said “safety is the number one priority” and the stadium complied with all safety regulations at the time of the incident.
The club also said “new configurations on the staffing and stewarding” of exits were introduced afterwards.
